Note : * KINDLY SURRENDER TRANSCO ID UPON CLAIMING SEPARATION PAY * A representative may claim in behalf of the claimant provided he/she presents a Special Power of Attorney and government issued IDs of the claimant and of the representative (i.e., GSIS e-card, driver’s license, passport, SSS ID, PRC ID). * An Affidavit of Undertaking must be also submitted before the checks are released; please coordinate with your regional HR group to secure a copy of such affidavit or download it from this website. * Non-submission of Affidavit of Undertaking-No release of check.
