2) A representative of the claimant will be allowed provided the former presents all of the following:

• A valid Special Power of Attorney
  
  
• a government-issued ID of the claimant and of the representative (e.g., driver’s license, passport, GSIS E-card, SSS ID, PRC ID).

• An Affidavit of Undertaking
